Annual Meeting of the Delhi State Chapter of IAPS was organized on the 25th of August 2007 at the Lady Hardinge Medical College. Dr. S.Roy Choudhury, Prof & Head, Ped. Surgery, LHMC, was the local organizer. The meeting was attended by 40 members. The guest of honor for the event was Prof. G.K.Sharma, Principal of LHMC. In his inaugural speech, he mentioned the long history of the Pediatric Surgery Department at the LHMC and its many contributions to the field of Pediatric Surgery. He highlighted how many stalwarts in the field had contributed their service here and how the tradition is being very well maintained today by a relatively young generation of Pediatric Surgeons.
There were two exciting and thought provoking presentations in the clinical meeting. The first presentation was “Esophageal preservation by esophageal lengthening procedures for esophageal atresia”. Three cases of long gap esophageal atresia managed by the above mentioned procedure were presented. A combination of Kimura’s extrathoracic elongation technique for lengthening of the upper pouch and Foker’s extrathoracic elongation for the lower pouch was applied, followed by esophageal end to end anastomosis. Although it was possible to restore the esophageal continuity, however, they required several operations, dilatations and a very high incidence of stricture formation requiring revision surgery. The second presentation was “MAS in children: an initial experience”. Different laparoscopic procedures like lap orchidopexy, lap hernia repair, lap appendectomy, lap cholecystestomy, lap varicocele ligation and few other procedures were presented to showcase the experience of the department.

Secretary’s report - I welcome you all to this Annual General Body Meeting of the Delhi Chapter of Indian Association of Pediatric Surgeons and it is my privilege and humble duty to present before you the annual report of the association. I begin the report by the encouraging fact that we have been able to hold the clinical meetings at regular intervals and in the two years of office of this team of office bearers, we have been able to hold 12 meetings and the attendance in most of the meetings has been good. The last clinical meeting was held at Chacha Nehru Bal Cikistsalya, Geeta Colony, in fact, had very good attendance. I must put on record the efforts made by all the executive members and our chairperson Dr. Arvind Sabarwal, who with her zeal, dedication and unique style, have been able to get together the act of organizing the meetings and more importantly to get people to attend them. Though most of the meetings were well attended, still I would urge to you my friends, to participate more eagerly in the clinical meetings and give suggestions to make them more informative and interesting. The Delhi Chapter of IAPS over the last 8 years has gradually increased its strength to 69 members with 3 new members joining in this year. Still a few of our colleagues who are not members of the chapter, I hope that they do become members during this year. The most important event of the year was organization of 20th Asian Association of Paediatric Surgeons Conference under the aegis of Delhi State Chapter of IAPS. We take this opportunity to congratulate Dr. D K Gupta for bringing such a prestigious conference and laurels to our chapter. Similarly we are trying to get the future meeting of World Congress of the World Federation of Association of Pediatric Surgeons under aegis of Delhi Chapter and solicit co-operation from all of you. We are delighted to know that WOFAPS has agreed in principle in their preliminary meeting to hold III WOFAPS conference in Delhi in 2010. With these few words, I now place the Statement of Accounts for the year 2006-2007 before the General Body for verification and approval.

Announcement of the Election results – The nominations for the post of Chairperson and executive committee members were invited this year on 31 May 2007. The last day to submit the nomination papers was 30 Jun 2007. We received only 2 names for the post of chairperson. No body applied for the post of executive committee members. Col Rakesh Handa and Col Ravi Kale applied for the post of Chairperson and later on Col Ravi Kale withdrew his name and Col Rakesh Handa was elected unopposed as the Chairperson for year 2007 – 2009. We congratulate Col Rakesh Handa and request members to suggest 5 names for the post of executive committee members. Following five names were suggested for executive committee - Col (Dr) Bipin Puri; Dr. Deepak Bagga; Dr Sandeep Agarwala; Dr Thichen Lama; Dr Chabi Ranu Gupta.

1. Single subject clinical meeting – He suggested that we should have one topic / theme for every clinical meeting. The local convener would liase with the other institutions and have them join in the presentations so as to complete the discussions on the selected subject
2. Contributory dinner along with families to be held once in a year to increase social interaction within our fraternity - Agreed
ii. Point from DK Gupta – Single topic discussions would limit the discussions to 6 topics per year since we have a bimonthly meeting. It should be left to the local organizer as to whether a single topic discussion or a multifaceted, colorful discussion is to be held.
iii. PESI Conference 2008 – The 3rd Annual Conference of the Pediatric Endoscopic Society of India is going to be held on 28-29 Mar 2008. The meeting is to be held at Max Super Speciality Hospital, Saket, New Delhi with Dr Meera Luthra as the Organising Secretary / Chairperson, Everyone was requested to contribute their might and participate actively
iv. Point from Dr Amita Sen – She highlighted how accrual of CME points to the tune of 60 hours per year will soon become essential for continuing medical registration. She suggested that we initiate measures to get our regular bimonthly meeting which lasts for about two hours accredited by the Delhi Medical Council.
v. Point from Dr Sujit Chowdhary – Informed the gathering how the IAP is holding a meeting on Disaster Management as related to children for which they have liased with the government and even received funds. There is no representation from Pediatric Surgery in the organization of this meeting. The house agreed how we should approach and convince the Govt that it is Pediatric Surgeons who are responsible for management of disaster related trauma in children and not Paediatricians. Dr DK Gupta suggested a two member committee to be formed to take up the matter of this disaster meeting being organized by the IAP and that we should make our presence felt at National/State level
vi. Welcome address by the new President Dr (Colonel) Rakesh Handa: He stressed on how we all should ensure regular attendance, including that by residents and our younger colleagues at these meetings to strengthen our Chapter. He also stated that in the matter of academic discussions we should be apolitical and should be united in increasing the impact and awareness of Pediatric Surgery. He requested all the members to work together for the successful organization of the 3rd PESI which is the next major challenge to be surpassed by the Delhi Chapter of the IAPS.

Minutes of the Annual General Body Meeting and Clinical Meeting on 20-9-2008 held at Chacha Nehru Bal Chikitsalaya
1. Annual Meeting of the Delhi State Chapter of IAPS was organized on the 20 September 2008 at Chacha Nehru Bal Chikitsalaya, Geeta Colony, Delhi. Dr. Anup Mohta, Prof & Head, Ped. Surgery, CNBC, was the local organizer. The meeting was attended by 30 members.
2. An introduction to the services provided by Department of Pediatric surgery, Chacha Nehru Bal Chikitsalaya, Geeta colony, Delhi was presented by Dr Anup Mohta, HOD and local convener.
3. Following three interesting cases were presented and discussed :
a) Hydatid lung or empyema- Diagnostic Dilemma (by Dr Sujoy Neogi): An eight year old child presented with history of abdominal pain and a tender lump in abdomen. There were no respiratory symptoms. On examination, left lung had absent breath sounds. Investigations were suggestive of hydatid lung. At thoracotomy, it turned out to be empyema lung. The case was discussed because of its unusual presentation.
b) Congenital anterior urethrocutaneous fistula associated with duplication of urethra (by Dr Swarup Das) : Congenital urethral fistula and duplication of urethra are uncommon congenital urethral anomalies. Cases where the two anomalies co-exist are very rare. We report an eight year old child where congenital urethral fistula and duplication of urethra were present along with the normal glanular urethral meatus.
c) Spontaneous small bowel evisceration in an infant (by Dr Mamta Sengar): A 4 month-oldgirl child presented to the emergency with history of prolapse of small bowel through anus following a bout of coughing. The child presented in shock and the prolapsed bowel showed areas of perforation and gangrene. Child was adequately resuscitated and an emergency laparotomy was planned. The abdomen was explored through Pfannensteil incision and the prolapsed bowel was reduced by traction-pulsion technique. Resection of the nonviable part of ileum was done along with creation of stomas. The child had a cardiorespiratory arrest 6 hours after surgery and expired. No case of small bowel evisceration has been described in literature during infancy. The purpose of this case report is to highlight the rare and life threatening complication of spontaneous evisceration of small bowel associated with prolapse of rectum in infants. All the three cases generated a lot of academic discussion.
4. The General body meeting began at 4.00 PM
a) Dr Rakesh Handa, Chairperson, welcomed all to the annual GBM. He announced the results of the elections to the post of Secretary cum Treasurer. There was only one nomination and Dr Anup Mohta was declared elected as the next Secretary cum Treasurer of IAPS Delhi Chapter for a period of 3 years (2008-2011). He complimented Dr MM Harjai for the excellent work done during his tenure.
b) Dr MM Harjai presented the Secretary’s report for the current year. Various activities organized under the banner of IAPS Delhi were informed. “I begin the report by the encouraging fact that we have been able to hold the clinical meetings at regular intervals and in the three years of my office, we have been able to hold 18 meetings and the attendance in most of the meetings has been good. In the last one year, we have had some excellent clinical meetings, one of which was held along with spouses at Vasant Vihar Club and Dr Sujit Choudhary had done an excellent job in organizing it. Another clinical meeting was held in Base Hospital Delhi Cantt, where again the trend of get together along with spouses was maintained. We held three important national events this year under the aegis of Delhi State Chapter of IAPS :
i) 3rd National Workshop on Paediatric Laparoscopy (PESI - IAPS 2008) was held at Max Super Speciality Hospital, Saket, New Delhi on 28th – 29th March 2008.
ii) Paediatric Urodynamics Workshop 2008 held at Maulana Azad Medical College, New Delhi on 30th March 2008, excellently organised by Dr YK Sarin and conducted by Dr Sripathi and Dr Sudipta Sen.
iii) Symposium on “Stem Cell applications - Promises and Potentials” held at AIIMS, New Delhi on 17 August 2008.
This is our sixth and final clinical meeting at Chacha Nehru Bal Chikitsalaya, Geeta Colony, Delhi which Dr Anup Mohta has organized very well. Though most of the meetings were well attended, still I would urge to all of you to participate more eagerly in the clinical meetings and give suggestions to make them more informative and interesting. This year no new member has joined, hence our strength remains the same. Still a few of our colleagues who are not members of the chapter, I hope that they do become members during this year.
Dr. D K Gupta is all set to bring prestigious World Congress of the World Federation of Association of Paediatric Surgeons under the aegis of the Delhi Chapter in 2010. We all wish him the very best in organising this Congress and assure him that the Delhi Chapter will provide all the help possible. Points from members: a) Dr Rakesh Handa pointed out that there was extreme difficulty in organizing clinical meeting for the month of May and July 2008 due to various reasons like vacations in the Government institutions. It was decided to make out a roster in advance and asked the institutions to opt for a certain month in advance b) A suggestion for CME accreditation for the IAPS clinical meeting by DMC was put forward by Dr Amita Sen
c) It was suggested that rather than single case reports, the presentations should also be regarding a series of cases from an institution. If necessary inputs from other Institutions may be sought.
d) Dr V Bhatnagar suggested organizing “Thematic meetings” with contribution from different Institutions, if required, with the local convener as coordinator.
e) A proposal to discuss difficult cases from institutes other than host Institute was discussed. It was informed this is being practiced even at present with a problem case being presented occasionally
f) It was suggested that there should be a “Scientific meeting coordinator”, but the consensus was that the Secretary IAPS Delhi will take that responsibility.
g) Dr Amita Sen invited all to the November meeting of IAPS Delhi Chapter on Friday, November 14th 2008 from 3.00 to 4.30 pm to be organized by Dr RML Hospital. Details of the meeting to follow.
h) A tentative schedule for the clinical meetings to be organized in the next year was proposed as following i. November 2008 : Dr RML Hospital. ii. January 2009 : Base Hospital Delhi Cantt iii. March 2009: : MAMC iv. May 2009 : LHMC/KSCH v. July 2009 : Max Super Speciality Hospital vi. September 2009 : AIIMS
i) Dr Bipin Puri raised the issue of modes of utilization of the money in IAPS account.
j) Dr. V Bhatnagar suggested that we should all make an effort for the corpus be raised and recommended that a journal by the Delhi IAPS be started.
k) Dr Satish Agarwal suggested starting a prize for the best paper in all the bimonthly meetings of the year. Members further suggested that the best paper award could be given at the annual meeting which could be a day long affair and may have a small workshop along with.
l) Dr Simi Rattan proposed to take steps to promote the speciality of Pediatric Surgery through the Media. Dr Amita Sen suggested to organize Public lectures for disseminating information to general public. Dr Anju Gambhir informed that one TV Channel has a slot for such proposal. Dr Shilpa Sharma suggested organizing Medical Camps for general public. Dr Simmi Rattan was nominated Public Relations Officer for coordinating such activities. Proposed by Dr. V Bhatnagar and seconded by Col Man Mohan Harjai.
m) It was suggested that more new members be inducted in to the fold of Delhi IAPS. n) Dr DK Gupta informed the members about the various activities and fellowships available under the banner of SAARC, Asian and World Federation for the promotion of Pediatric surgery. Minutes of the clinical meeting held on 22.11.2008
The clinical meeting for the month of November was held at Panchsheel club, New
Delhi. The meeting was organized by Max superspecialty Hospital and the local
coordinator was Dr Meera Luthra.
The meeting was attended by 30 members.
Following cases were discussed:

All the cases generated good discussion and the suggestions were given regarding the
management options for the difficult cases.
Dr Simmi Ratan, Assistant Professor at Department of Pediatric surgery, Maulana Azad
Medical College who accepted the responsibility for finding ways to spread awareness
regarding about the specialty in the public gave the progress made since the meeting in September. She informed that she has met the concerned officials in the Ministry of
Health at Centre and Delhi Govt.
The meeting was preceded by Lunch and beverages.
It was announced that the next clinical meeting will be held at RR Hospital to be
coordinated by Col B Puri. The meeting ended with a note of thanks.

Minutes of the clinical meeting held on 18th January 2009
First monthly meeting of Indian Association of Pediatric Surgeons ( Delhi chapter) was held on 18th January 2009 at Research and Referral Hospital, Dhaula Kuan. Local coordinaror was Col Bipin Puri. It was attended by 20 members and 12 guests from R&R hospital. Cases discussed were
a) Hydatid cyst lung- presented by Maj. Manvendra Jha
b) A case of crossed fused renal ectopia- presented by Maj Govindji Tahkur
c) A case of urogenital sinus and patent urachus- presented by Maj BK Biswas The cases generated good academic discussion about management strategies.
Dr Simmi K Ratan apprised the members of the efforts made to popularize the specialty. Posters and other modalities were discussed. The meeting ended with a vote of thanks and lunch.
2. Clinical meeting for March 2009 Clinical meeting for March 2009 will be organized by Department of Pediatric surgery, Maulana Azad Medical College on 7th March 2009. Local coordinator is Dr SK Aggarwal.

6. CME Department of Pediatric surgery, Chacha Nehru Bal Chikitsalaya (affiliated to Maulana Azad Medical College) is organizing a CME on Current Practices in Pediatric Surgery on Sunday, 15th March 2009. The target audience is general surgeons, especially the post graduate residents in surgery. There is no registration fee.

Minutes of the Clinical meeting on 7th March 2009
Clinical meeting for the month of March 2009 was organized by Department of Pediatric surgery, Maulana Azad Medical College, New Delhi. Local coordinator was Prof Satish K Agarwal. The meeting was attended by 27 members and few invited guests. The members were greeted with colors of Holi courtesy Dr Simmi Ratan and a sumptuous lunch. The cases presented were
1. 156 cases of Laparoscopic surgery at Maulana Azad Medical College- presented by Dr Nitin Pant and discussed by Dr Satish K Agarwal.
2. Genitourinary injuries in girls-11 cases- presented by Dr Nitin Pant
3. Massive hemoptysis in a 9 year old girl- presented by Dr Rupnayan Goswami and Dr Anjan Dhuia
4. Urological complications in anorectal injuries
All the presentations generated good clinical discussion especially on the variety of laparoscopic surgery being performed at MAMC as well as the management of genitourinary injuries
2. CME on Current Practices in Pediatric Surgery A CME “ Current Practices in Pediatric Surgery” was organized by Department of Pediatric surgery, Chacha Nehru Bal Chikitsalaya under the aegis of Indian Association of Pediatric Surgeons (Delhi Chapter) on 15th March 2009. The target audience was general surgeons, especially the residents in general surgery. There was no registration fee. Dr Anup Mohta was the Organized Secretary. The CME was a great success and was attended by about 75 delegates. All the topics that are relevant to postgraduate students for their final examinations were discussed. A CME book was also released on the occasion.

After a gap of three months on account of vacations, its time to start our clinical meetings again.
1. Minutes of the meeting held on 28-04-2009
Clinical meeting was held at All India Institute of Medical Sciences on 28-04-2009. The clinical meeting of the Association was preceded by a CPC presented by Department of Pediatric surgery AIIMS. Prof Ashoke Basu, President Elect IAPS, from Kolkata was the discussant. An interesting case of abdominal lump in a child was discussed by Faculty of Pediatric surgery, Pathology and other clinical departments. This was followed by the clinical presentations. Four cases were presented.
1. A case of Neonatal Airway Obstruction
2. A 10 year old child with mass in the lower jaw- ossifying fibroma
3. A child with pneunectomy
4. A case of Pleuropulmonary blastoma
Following this, Prof DK Gupta informed the members about the preparations going on for the World Congress of Pediatric Surgery going to be organized at New Delhi in 2010. 2. Clinical meeting for August 2009 Next clinical meeting for the month of august will be held at Lady Hardinge Medical College and Kalawati Saran Children’s Hospital on Saturday 29th August 2009. Annual Clinical meeting and General Body Meeting on 12-09-2009
Annual clinical meeting started with presentation of clinical cases by Department of Pediatric Surgery, Dr RML PGIMER, New Delhi. Dr Amita Sen was the local convener. The meeting was also attended by Dr NK Chaturvedi, a renowned pathologist and Medical superintendent, Dr RML PGIMER along with other faculty members of the Institute as guests. Following cases were presented.
1. Case of Carcinoma colon in a child - Dr Sudhir Singh
2. Mediastinal mass in children - Dr Vipin
3. Experience with xcases of annular pancreas - Dr Mayank
4. Experience with trichobezoar - Dr Nusrat Jahaan
This was followed by presentation of a difficult and interesting case of anorectal malformation being managed by Dr Rakesh Handa. All the cases generated good academic discussion.
General Body Meeting
1. The GBM started with the welcome speech by Dr Rakesh Handa, President, IAPS Delhi State Chapter. He outlined the activities of the Association.
2. Announcement of election results: i. Elections were called for the post of President and 5 executive members of the Association. ii. Only one nomination from Dr Deepak Bagga was received for the post of President. He was declared elected unopposed to the post of President for the next two years. iii. No nomination was received for the post of executive members. Nominations were called from the house. Following members were declared nominated to the executive committee a) Dr Satish K Aggarwal ( MAMC) b) Dr Shilpa Sharma ( DR RML PGIMER) c) Dr Archana Puri ( LHMC and KSCH) d) Dr Alpana Prasad ( SGRH) e) Dr Dharmender Singh ( Max , Patparganj) The house welcomed the new executive members.
3. Presentation of the Secretary’s Report and financial statement by Dr Anup Mohta, Secretary cum treasurer I welcome you all to the General Body Meeting.
1. Activities held during the year 2008-09 a) Clinical meetings: Clinical meetings were held at different centers. (i) Max Hospital on 22nd November 2008 (ii) RR Hospital on 18th January 2009 (iii) MAMC on 7th March 2009 (iv) AIIMS on 28th April 2009 (v) Kalawati Saran Children’s Hospital on 29th August 2009 and (vi) Dr RML PGIMER on 12th September 2009 All the meetings were well attended and good clinical material was presented. b) A CME on “Current Practices in Pediatric Surgery” under the aegis of IAPS (Delhi Chapter) was organized by Department of Pediatric surgery, Chacha Nehru Bal Chikitsalaya on 15th March 2009. The CME was targeted at the postgraduates in surgery and general surgeons and was well attended.
2. New members The Association was able to enroll 14 new members during the current year. This was quite encouraging but the momentum has to continue. There are still many more residents in different Department who have to become members of IAPS- both Cenrtal and Delhi Chapter.
3. Future activities a) Dr Sujit K Chaudhary is the Organizing Secretary of the International Children's Continence Society meeting on 5,6 Dec'09, India Habitat Center, Lodhi Road, New Delhi. b) Prof DK Gupta is the Organizing Secretary of the World Congress of Pediatric Surgery on October 21-24, 2010 and the venue is Hotel Taj Palace and convention centre. There is a Post Congress Workshop on OCT 25-26, 2010 AT AIIMS.
4. Initiatives for the promotion of Pediatric surgery a) Dr Simmi K Ratan has made sincere efforts to promote the cause of Pediatric surgery and has held many meetings with the higher officials b) Dr Anup Mohta met the Principal Secretary (Health), Govt. of NCT of Delhi and apprised him of the present scenario of Pediatric surgery facilities in the Capital. The efforts will continue and the follow-up will be intimated. I thank Col( Dr. Rakesh Handa, our President for all the support and guidance in running the activities of the Association. He will be relinquishing his charge in this meeting but I am sure he will be available for his advise. I also wish to thank all the EC members for co-operation. I also thank all the Senior members for their guidance at many occasions. I assure you that I shall continue to work for the Association with the incoming President and the new Executive Committee.

4. Roster for the clinical meetings. As the number of centers willing to host the clinical meeting has been increasing, it was felt that the frequency of the clinical meeting needs to be increased. Some senior members were not in the favour of monthly meetings as the number of members attending was not enough, many others suggested that the number may be increased to 10 meetings per year so as to accommodate more centers. A roster may be chalked out with discussion.
5. Registration of Association Secretary sought the opinion of the house as to need and mode of registration of the Delhi State Chapter. It was decided that the matter may be deferred at present as the Association membership is not large enough and there are not much financial assets so as to warrant registration under present situation.
6. Any other matter with the permission of the Chair
a) Prof DK Gupta apprised the members of the currents status of preparation for the World Congress in 2010. Financial and organizational implications were also discussed. He urged the members to help generate funds. It was suggested by members that we can submit the registration fee for the Congress at the earliest by cheque in the name of “World Congress of Pediatric Surgery”. All the members are requested to send the cheque for Rs 10,000 to Prof DK Gupta directly or through any executive member.
b) Dr Simmi Ratan requested the Senior members to use their influence on the Senior government officials so as to promote the specialty of Pediatric surgery.
7. Dr Rakesh Handa handed over the chair of President of IAPS (Delhi State Chapter) to Dr Deepak Bagga amidst applause by the members. Dr Bagga thanked the members for reposing confidence in him and assured to work for improvement of specialty.
8. Vote of Thanks Dr Anup Mohta thanked all the members for attending the clinical meeting and making it successful.
CLINICAL MEETING HELD ON 31-10-2009
Clinical meeting for the month of October was held on 31-102009 at Vardhaman Mahavir Medical College and Safdarjang Hospital, New Delhi. The meetimg was preceded by a sumptuous lunch.
a) At the outset, Dr Shalini Aggarwal, was requested to draw the lot of the successful candidates from a pool of 70 early Bird Registrants for the World Congress of Pediatric surgery 2010 eg. 1 in 5 for the Banquet and the Workshop alternatively. b) This was followed by the presentation of the clinical cases. The cases presented were:

b) Anterior Urethral Valves with diverticulum: Dr Shantanu
c) Anterior Sagittal Transrectal Approach ( ASTRA) to female Dr Narinder urethral injuries
d) Laparoscopic Hernia repair: to do or not to do Dr Deepak Bagga Audience appreciated the clinical material presented and a lot of discussion was generated.
c) The next clinical meeting of the IAPS Delhi Chapter will be held in association with International Continence Society Conference to be held on 5-6th December 2009. The meeting is being organized at India Habitat Centre and Dr Sujit Chowdhary of Apollo Hospitals, New Delhi is the Organizing Secretary. He briefed the members about the meeting and registration fee for the IAPS Delhi chapter members.
d) New Appointments: Dr Shandip Kumar Sinha joined Department of Pediatric Surgery, Maulana Azad Medical College as Assistant Professor of Pediatric Surgery.
e) Honorary Secretary once again requested the residents to become the members of IAPS at central and Delhi chapter level.
